Authorization for Removal of Restricted or Sensitive Information

Prior to removing any Restricted or Sensitive Information from the UF/Shands Health Science Center premises, each individual needs to be authorized by the Psychiatry Information Security Administrator, Marika Brigham. 02/19/07 Laptop Security Memo.

This includes any Restricted or Sensitive electronic data on laptops, PDAs, USB memory, CDs etc. It also includes any Restricted or Sensitive information on paper documents.

To obtain authorization, fill and sign the Information Removal Authorization form below and fax it to the Psychiatry Fiscal Office

Information Classification Policy

All information created, stored, managed and/or destroyed by the UF Department of Psychiatry is classified as either Restrictive, Sensitive, Operational, or Unrestricted. The security procedures and storage considerations of the information is determined by its classification.

Psychiatry Information Classification is an official departmental document defining the above terms and explaining the distinctions between them.

Security Incident Response Policy

Department personnel are to report all concerns regarding information security to our Unit ISA Marika Brigham (Marika@psychiatry.ufl.edu, 352-265-7981) or Unit ISM John Glasheen (Glasheen@psychiatry.ufl.edu, 352-265-7981).
